Why breath matters in addiction recovery
The autonomic nervous system - which regulates heart rate, stress response, and many involuntary functions - is normally beyond conscious control. The breath is an exception: it is the only autonomic function that can be directly controlled by the conscious mind, which is why breath is the entry point used by virtually every contemplative tradition in human history to regulate states of body and mind.
This is particularly relevant in recovery for two reasons. First, chronic substance use dysregulates the autonomic nervous system - reducing parasympathetic (rest-and-digest) tone and increasing sympathetic (fight-or-flight) activation, creating chronic arousal and reactivity that persists into early abstinence. Second, the capacity for emotional regulation - managing difficult states without resorting to substances - is precisely what recovery requires, and conscious breath control is one of the most immediately accessible tools for developing it.
The physiology of breathwork
The vagus nerve
The vagus nerve is the primary conduit of the parasympathetic nervous system. Slow, deep diaphragmatic breathing - particularly with an extended exhalation - activates the vagus nerve and increases vagal tone: the degree to which the parasympathetic system is active. High vagal tone is associated with better emotional regulation, lower baseline anxiety, greater resilience to stress, and better social functioning. These are exactly the outcomes that recovery programmes aim to produce.

Heart rate variability
Heart rate variability (HRV) - the natural variation in time between heartbeats - is a reliable marker of autonomic nervous system health. Research has shown that people with addiction tend to have reduced HRV, and that HRV normalises with sustained abstinence. Slow resonance breathing at around 5 to 6 breaths per minute reliably increases HRV and is one of the fastest ways to improve this marker.
Carbon dioxide and the stress response

This is the approach most directly supported by physiological research. It is calming, grounding, and particularly useful for managing anxiety and craving states in early recovery.
Box breathing
Box breathing - inhale for 4 counts, hold for 4, exhale for 4, hold for 4 - is used to rapidly shift nervous system state. It is widely taught in military contexts for managing acute stress and translates well to recovery as a tool for interrupting craving peaks and emotional dysregulation.
Holotropic breathwork
Holotropic breathwork uses faster, deeper breathing sustained over longer periods to induce non-ordinary states. These can include vivid imagery, emotional catharsis, and experiences clients describe as deeply meaningful. The evidence base is primarily qualitative rather than from randomised trials, but many practitioners report significant therapeutic breakthroughs. It requires careful clinical management, particularly for people with significant trauma histories.
Somatic breathwork
Approaches drawing on somatic therapy traditions use breath in combination with body awareness and movement to release stored tension and emotional charge from the body. These are particularly relevant for people with trauma histories where the traumatic material is held in the body rather than in narrative memory.
Breathwork and craving management
Cravings are, at the physiological level, an activation of the arousal system - heightened sympathetic nervous system activity. Slow, controlled breathing is one of the fastest and most reliable ways to reduce this arousal. Teaching clients simple breathwork techniques during treatment creates a skill available in real-world situations after discharge. Unlike medication, it requires no prescription. Unlike calling a sponsor, it can be deployed privately and immediately - anywhere, at any time.
Building a personal practice
The benefit of breathwork is cumulative. A single session produces acute changes in nervous system state. A regular practice - even 10 minutes daily - produces lasting changes in baseline HRV, stress reactivity, and emotional regulation capacity. The most effective use of breathwork in recovery is as a daily practice, ideally established during residential treatment when structure supports habit formation.
The simplest effective practice: 10 minutes of slow, diaphragmatic breathing each morning, with the exhalation slightly longer than the inhalation. No equipment required. The evidence that this produces meaningful changes in the physiological systems most disrupted by addiction is robust.
